To help ensure a smooth process and to gain a greater understanding of your goals, it is important to obtain as much information as possible in the referral form, together with the NDIS plans.
Here is a handy checklist that will help process your form quickly.
NDIS Checklist before submission
- Review Services and Fees Guidelines
- Sign the last page
- All mandatory sections completed
- Attach NDIS plan or subset of Plan (goals and improved daily living budget)
- Previous therapy reports (if applicable)
What to expect after I have submitted referral form
You will receive a service agreement and this will outline the amount of hours required, the cost and details of the therapy service. This form can be signed at the first appointment or you can sign it prior and send it to email@example.com.
