The National Disability Insurance Scheme (NDIS) is a national program that enables people with a disability to choose their support services to meet their personal needs and goals.

National 360 and NDIS

National 360 is a Registered NDIS Provider of Occupational Therapy, Speech Pathology, Physiotherapy, Behaviour Support and Early Childhood Services in Victoria, New South Wales, Queensland, South Australia, Western Australia and Tasmania. Our therapies generally fall under activities of “Improved daily living or behaviour support” budgets within NDIS plans.

As an experienced NDIS service provider, we deliver a range of services across paediatrics including early childhood intervention, adult disability, aged care and mental health. 

Our occupational therapists, speech pathologists, physiotherapists, behaviour support practitioners and early childhood practitioners work closely with NDIS participants – along with family members, carers, NDIS planners and support coordinators – to collaboratively deliver a support program that helps our clients achieve their independence goals and receive the support they need.

As an Australia-wide NDIS agency, we have a team of dedicated support staff and local area coordinators who have expertise across the NDIS that can assist in processing referrals and allocating the best therapist to meet your needs. 

To help ensure a smooth process and to gain a greater understanding of your goals, it is important to obtain as much information as possible in the referral form, together with your NDIS plans.

Here is a handy checklist that will help process your NDIS referral form quickly.

NDIS Checklists before submission

  1. Review of Services and Fees Guidelines 
  2. Sign the last page
  3. All mandatory sections completed
  4. Attach NDIS plan or subset of Plan (goals and the relevant support budget)
  5. Previous therapy reports (if applicable)

What to expect after I have submitted a referral form

You will receive a consent form and service agreement, this will outline the number of hours required, the cost and details of the therapy and support services. This form can be signed at the first appointment, or you can sign it prior and send it to info@national360.com.au.

Yes – we are registered to provide services to NDIS participants in VIC, NSW, QLD, SA, WA, TAS and NT.

Our therapists are located in multiple locations throughout regional and metro Victoria, New South Wales, Queensland, South Australia, Western Australia and Tasmania. This enables us to match our clients with highly skilled clinicians in the required geographical location to ensure that the best clinical outcome is achieved.

We appreciate that an NDIS plan is a personal document, however by attaching a copy of your NDIS plan (or your goals and the relevant support budget) we can provide the most suitable therapist and ensure we are working within the funding available. The end result is a more seamless process and alignment to your needs.

After we receive your referral details, we will confirm with you which services you would like us to provide, the agreed cost and time frame. A therapist will then contact you to make time to visit.
