The Power of Play: How Ameer Is Building Skills and Communication with 360 Kids

A Transdisciplinary and Multidisciplinary Approach to Progress

360 Kids Early Childhood Supports - Ameer's Story Ameer is a fun and happy 8-year-old who is supported at 360 Kids by National 360 clinicians through our Mixed Model Service Approach. Ameer lives with developmental delays that impact many areas of his daily life, including his communication, mobility, self-care, and participation in everyday activities. Ameer and his family reside in Toowoomba, where access to allied health services is limited due to the regional location and workforce availability. As a result, timely access to specialist supports can be challenging, highlighting the importance of coordinated, flexible service delivery.

The Mixed Model Service Approach combines transdisciplinary and multidisciplinary supports to meet Ameer and his family’s individual needs. A Transdisciplinary Approach included allocating a lead clinician who supports Ameer to work towards goals across multiple allied health areas, drawing on guidance from the broader allied health team. A Multidisciplinary Approach involves one or more other clinicians also providing direct therapy alongside the lead clinician when their specific expertise is required.

Through the Mixed Model Service Approach at 360 Kids, Ameer and his family receive coordinated support from a team of clinicians who work together to achieve shared goals. This model allows therapy to be flexible, connected, and responsive as Ameer and his family’s priorities evolve.

Angela, Ameer’s Early Childhood Practitioner, is his Lead Clinician and provides weekly face-to-face therapy sessions. With guidance and support from the wider therapy team, Angela works with Ameer on Speech Pathology, Physiotherapy, and Occupational Therapy goals during everyday activities and routines.

Ameer’s Physiotherapist, Ricky, visits periodically to review progress and provide specialised input, while his Occupational Therapist, Olivia, and Speech Pathologist, Emily, provide ongoing support remotely. The team works closely together, sharing strategies, planning collaboratively, and ensuring that therapy recommendations can be incorporated into Ameer’s daily life.

An important benefit of this approach is that Ameer’s family has one main point of contact. Rather than coordinating between multiple clinicians, the family can liaise directly with Angela, who works closely with the wider team. This reduces overwhelm for the family, simplifies communication, and helps ensure that everyone is working towards the same goals.

By combining the expertise of multiple clinicians through one coordinated approach, Ameer’s family receives consistent support that is practical, meaningful, and focused on helping him participate more fully in the activities that matter most to him.

Ameer has been having lots of fun building his core strength, motor skills, and body awareness through active play. We’ve also been supporting his communication by helping his family use Key Word Sign and respond to his communication during everyday routines. Ameer is making great progress -he’s getting stronger during movement activities and is now pointing to visuals and copying Key Word Signs to let others know what he wants and needs. – Angela, Early Childhood Practitioner
