How to Access Support at Home with National 360

  1. Submit an aged care referral: Begin by completing the aged care referral form on our website. This helps us understand your needs and the type of supports you’re looking for and it can be completed by your aged care provider or by clients and families who have the relevant information. 
  2. We contact you and book your first appointment: Our Client Support Team will be in touch to discuss your referral, answer any questions, and book your initial appointment.  
  3. In-home assessment or first service: A clinician visits your home to understand your goals, environment, and the supports that will help you stay safe and independent. 
  4. Ongoing therapy and support from there, you’ll receive regular therapy sessions, progress updates, and adjustments to your plan as your needs evolve.
